I, the undersigned participant (or parent/guardian of the participant if under 18 years of age), understand that participation in Onward Students Events comes with inherent risks, including but not limited to injury, illness, or property damage. I acknowledge that Onward Students and its leaders, staff, volunteers, and agents will take all reasonable precautions to ensure the safety and well-being of the participants.

In consideration of being allowed to participate in this event/activity, I hereby release, waive, discharge, and hold harmless Onward Students, its leaders, staff, volunteers, and agents from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant, or to any property belonging to the participant, while participating in the event/activity.

I understand that this waiver includes any claims based on negligence, action, or inaction of Onward Students, its leaders, staff, volunteers, and agents.

Medical Treatment Authorization *

In the event of an emergency, I authorize Onward Students to secure any necessary medical treatment for the participant. I agree to be responsible for any costs related to such treatment.

Assumption of Risk *

I understand and acknowledge that participation in the event/activity may involve risk of injury. I assume full responsibility for any risk of bodily injury, death, or property damage arising out of or related to the event/activity.

Photo/Video Release

I hereby grant Onward Students permission to use photographs, videos, or other media taken of the participant during the event/activity for promotional purposes, including but not limited to social media, websites, and printed materials. I understand that these images may be used without further notification or compensation.
